About the Role

As a District Nurse Specialised, you will play a pivotal role in undertaking admissions and conducting comprehensive clinical assessments for both new and existing clients. You will analyse complex client situations, ensure care activities align with established protocols, and actively engage clients and their carers in their care processes. You will monitor and evaluate client progress, maintain accurate records, and update care plans proactively to ensure the effectiveness of nursing interventions.

Responsibilities include:

  • Undertakes admissions and conducts clinical assessments for clients, referring to Clinical Nurse Consultants or Allied Health Professionals when needed.
  • Ensures integrated client care activities are performed using appropriate protocols and promotes client and care giver involvement in care processes. Establishes and maintains therapeutic relationships with clients and care givers.
  • Analyses client situations, integrates clinical data from various sources, maintains client records and makes nursing decisions in complex cases and seeks senior assistance as required.
  • Participates in professional development and quality management initiatives to enhance clinical care delivery standards and provides nursing care in adherence with relevant legislation and common law.

About You

To be successful in this role, our preferred candidate will have:

  • Bachelor’s Degree required; Nursing, Healthcare is preferred. Any other related discipline or commensurate work experience considered.
  • Minimal experience as a registered nurse in a healthcare setting is preferred.
  • Registered Nurse Division 1 Certification is prescribed.
  • Strong knowledge of Quality Care Service Management, Administrative Support, Patient Case Management and Clinical Practices and Standards.
  • Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills.
